I hereby give my full consent to receive a tattoo performed by the named artist. I acknowledge that:

• Tattooing involves inserting pigment into the skin using needles and may cause pain, swelling, bleeding, or discomfort.

• Potential risks include, but are not limited to, infection, allergic reactions, scarring, and variations in the final appearance.

• The final result may differ from the initial design due to factors such as skin type, healing, sun exposure, and aftercare.

• Complete tattoo removal may not be possible and may result in scarring or skin discoloration.

• I have received and understand the aftercare instructions provided and will follow them carefully to minimize risks.

• I have been advised of the signs of infection and understand when to seek medical attention.

I understand and accept all potential risks associated with tattooing. I am voluntarily choosing to undergo this procedure and release the artist and studio from any liability arising from complications or results of the tattoo.
